Prompted by a 2010 decree-based expansion of community living and services in Illinois, our Comprehensive Class Member Transition Program transitions people with disabilities/mental health challenges from restrictive institutional environments to community-based living. Staff provide a variety of supports, such as care plan development, housing and transition assistance, counseling, skill building services, and other supports to meet client-specific needs. We know that people with disabilities, with the right services and supports, can thrive in community-based settings with choice, independence, and inclusion.
