We support adults transitioning from state-operated psychiatric hospitals back into the community. Our services include individual assessment, discharge planning, benefits assistance, and housing search — all designed to promote stability and reduce the rate of re-hospitalization.
Our Transition Support Specialists understand the challenges clients face during this critical period. They work closely with each individual to identify the services and supports needed for a successful, sustainable transition into community life.
